The upper or middle back is also referred to as the thoracic spine. It's anatomical design allows for stability to anchor the rib cage as well as protection of vital internal organs of the chest and upper abdomen. The upper back is remarkably resistant to injuries and pain as compared to the neck and lower back. Because of the stability of the rib cage the thoracic spine is less likely to incur a flexion/extension type injury. People often associate upper back pain to the term "stiff neck". This type of stiffness in the upper back often makes the neck and shoulders feel stiff or uncomfortable. Muscular irritation in the upper back is typically due to deconditioning or overuse of muscles.
